Application Terms & Disclosure By E-Signing below and clicking "Submit Application," you certify that all statements in this application are true and made for the purpose of obtaining credit. You authorize Long Island Auto Source to initiate a credit investigation, including obtaining your consumer credit report, and to forward your application to lenders, financial institutions, or other third parties for processing. We, as the auto broker, may collect information about you as described in this notice, which we handle according to our website privacy policy. If required, our official company privacy policy will be provided to you in person, by mail, or electronically with your consent. This notice does not apply to information obtained in non-financial transactions. All applicants must meet lender criteria to qualify for credit. Not all applicants, including pre-approved applicants, will qualify for a loan. By completing and submitting your request, you consent to receiving all legally required disclosures, notices, privacy policies, and information ("Disclosures") in electronic form and agree to conduct business with us electronically. We collect nonpublic personal information about you from various sources, including information provided on applications or other forms in connection with a financial transaction, details of your transactions with us, our affiliates, or others, and information from consumer reporting agencies. We do not sell customer or consumer information to any third party. We may disclose the information we collect, as described above, to our affiliates, other financial institutions with which we have joint marketing agreements, or companies that perform services on our behalf. These companies may include financial service providers and non-financial entities such as computer and data processing companies, and marketing and aftermarket service providers. These companies are contractually obligated to maintain the confidentiality of the information we provide and to use it only to provide the requested services. We comply with legal limits and restrictions on information sharing with our affiliates or others. Access to nonpublic personal information about you is restricted to employees who need to know that information to provide products or services to you. Delivery Authorization, Accident Waiver and Release of Liability I hereby authorize Long Island Auto Source and its directors, officers, employees, volunteers, representatives, vendors, and agents to pick up and deliver the agreed-upon vehicle to/from the dealership and to/from the agreed-upon delivery location. I assume all risks associated with having my vehicle delivered to the selected destination. In consideration of the additional delivery fee that the company waived, and in exchange for this release and the service provided, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: I waive, release, and discharge Long Island Auto Source and its directors, officers, employees, volunteers, representatives, vendors, and agents from any and all liability, including but not limited to liability arising from the negligence or fault of the entities or persons released. I further agree to indemnify, hold harmless, and not to sue Long Island Auto Source and its directors, officers, employees, volunteers, representatives, vendors, and agents for any and all claims related to or arising out of the delivery of my vehicle from its current location to the destination of my choosing. This Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I certify that I have read this document and fully understand its content. I am aware that this is a release of liability and a contract, and I sign it of my own free will.
